WHAT: In the third letter between Corporal Punishment and Havelock Stern, there appears to be a typo. In the second sentence, it says "qtjjy dkpjgjlq qmfjv qkcugjl rnfd..." when I believe it should say "qtjjy dkpjgjlq qmfjr qkcugjl rnfd...".
HOW: Start the CP/HS questline and continue to the third letter overall (the second from the Corporal to Havelock).

WHEN: I don't know - I just encountered it. I assume it's been there since that letter was originally written, though.
