WHAT In targeted fighting after stunning a monster then hitting the wrong arm so it unstuns, the system freezes. After that point nothing changes colors (go from unguarded to guarded or visa versa) no matter what you hit.

HOW Stun a monster then hit the wrong stun sequence.

WHEN I discovered it today, but I haven't been on for a while.
